Anyone living with eczema or psoriasis knows the routine: the tightness, the flaking, the itch that arrives at 2 a.m. And anyone who has stood in a skincare aisle knows the second problem, a hundred products claiming to be "gentle," with very little to distinguish them.

When filaggrin is scarce, the barrier becomes more permeable. Water escapes outward trans-epidermal water loss (TEWL) — while irritants, allergens, and microbes find it easier to get in. The immune system responds, and the familiar cycle begins: itch, scratching, more barrier damage, more inflammation.
This is why the National Eczema Association treats consistent moisturizing as a foundation of daily care. It's also why moisturizing alone is rarely enough during an active flare, the inflammation itself usually needs to be addressed medically.

Emollients support stratum corneum integrity, reduce trans-epidermal water loss, and can lower flare frequency when used consistently.
Those same guidelines continue to list topical corticosteroids as first-line therapy for flares, alongside a growing set of non-steroidal prescription options: topical calcineurin inhibitors, PDE-4 inhibitors, and topical JAK inhibitors, with biologics and systemic agents for moderate-to-severe disease.
So the accurate framing is not plant-based instead of medical treatment. It's this:
Daily barrier care is the part you do every day, forever. Medical treatment is what you and your clinician use to bring inflammation under control. They work together, and good daily care can meaningfully reduce how often you need the second one.

- Added fragrance and fragrance blends — the leading cause of allergic contact dermatitis in cosmetics, and the single most useful thing to eliminate.
- Essential oils at functional concentrations — lavender, tea tree, peppermint, and citrus oils contain sensitizing terpenes. "Natural" does not mean low-risk here; if anything, the opposite.
- Isothiazolinone preservatives (MI/MCI) and formaldehyde releasers — these are the preservatives that actually dominate contact allergy registries.
- Denatured alcohol high in the ingredient list — degreasing and drying on already-compromised skin.
A correction worth making, since it circulates widely: parabens and phenoxyethanol are among the lowest-sensitization preservatives in cosmetic use. They are not the problem ingredients they're marketed as. Avoiding them is a formulation-philosophy choice, not a barrier-health one.
A Daily Barrier-Care Routine
This is the supportive routine, the part that runs alongside whatever your clinician has prescribed.
1. Cleanse Without Stripping

A word on the "soak and smear" technique, since it's often described inaccurately: the original method involves a plain-water soak of roughly 20 minutes, followed immediately by an ointment applied to still-wet skin — and in its original clinical description, that ointment typically contained a prescribed topical steroid. If you want to use it, do so as your clinician directs, not as a serum-then-cream sequence.
3. Layer Sensibly: Humectant → Cream → Occlusive
- A humectant layer (glycerin, hyaluronic acid, aloe) draws water in.
- A lipid-rich cream replenishes the barrier and holds that water.
- On the driest, roughest, or most fissure-prone areas, a waterless balm on top provides the final seal.
Smooth products on rather than rubbing vigorously — friction generates heat and provokes the itch reflex. On psoriatic plaques, press product in gently and give it time; softening is gradual.

- Laundry: fragrance-free detergent, and skip liquid softeners and dryer sheets — the residue sits against skin all day.
